1 Policy Statement
To recognize and/or reward individuals, associations, companies or agencies who have contributed to the success of the SESHA or have contributed to the environmental health and safety fields within the semiconductor industry.
2 Scope
The following policy statement and enclosed appendices define the specific methods by which SESHA may recognize and/or reward such achievement.
3 Selection and Approval Process
The selection and approval process is dependent on the level of recognition. See attached appendices for:
- Certificate or Plaque of Appreciation

- President’s Award
- Fellowship Award
- Lifetime Achievement Award
4 Who Can Nominate

6 The Award
The award recipients are entitled to receive various benefits depending upon the level of the award. See attached appendices.
Lifetime Achievement Award
SESHA
Effective Date – January 1997
1 Policy Statement
To recognize a uniquely qualified individual who has dedicated his/her career to and significantly impacted the fields of of environmental health and safety in the semiconductor industry.
2 Scope
The following policy statement defines the specific method by which individuals are selected for and approved by the SESHA Board of Directors for the SESHA Lifetime Achievement Awards.
3 Selection and Approval Process
The following are recommended qualifications for an individual to be nominated to the SESHA Board of Directors for the SESHA “Lifetime Achievement Award”:
3.1 The individual must have significantly impacted and contributed to the semiconductor industry in the fields of environmental health and safety.
3.2 This award is given for a life time of dedication and achievement and is to recognize a selected individual (living or dead). It is usually given at the end of one’s career as one is retiring or leaving the field after a minimum of 20 years of service. In the case of a deceased nominee, the minimum 20 year service time may be waived and the award given to his/her heirs.
3.3 This award is to be restricted to a maximum of one individual per year to maintain its significance and uniqueness. However, the SESHA Board of Directors has the discretion to pass on granting this award in a given year, if a qualified individual is not available.
3.4 A nominee for the SESHA “Lifetime Achievement Award” must have been an SESHA member and should hold the designation of SESHA “Fellow”.
4 Who Can Nominate
4.1 Any current SESHA member can nominate an individual to the SESHA Board of Directors for the SESHA “Lifetime Achievement Award”. The nomination must be in writing and include the nominee’s qualifications and the reasons why he/she is to be considered. The SESHA Board of Directors may also appoint a nominating committee to seek qualified individuals for this prestigious award.
5 Responsibilities
5.1 The SESHA Board of Directors will review the qualifications, rank, rate and vote on all nominees for the SESHA “Lifetime Achievement Award” on an annual basis. The nomination will be carried if a 2/3 majority of the Board is achieved on the vote. The SESHA Board of Directors may delegate this responsibility to a sub-committee appointed by the Board.
6 The Award
6.1 The recipient of the award will receive the following:
-
- All accommodations (transportation, hotel, meals and registration fee) to the SESHA annual meeting for the award winner will be provided by SESHA.
- A recognition plaque and presentation will be made at the annual meeting by the current SESHA President.
- The award recipient will have one of the SESHA Scholarships named after him/her.
- An article detailing the accomplishments of the award recipient will be published in the SESHA Journal.
- The SESHA “Lifetime Achievement Award” recipient receives all the privileges of an SESHA “Fellow”.
Fellowship AwardSESHAEffective Date – January 1997 1 Policy Statement
To recognize uniquely qualified individuals who have significantly distinguished themselves in the eyes of the SESHA Board of Directors by academic accomplishments, technical achievement, or meritorious service in the disciplines of environmental health and safety within the semiconductor industry.
2 Scope
The following policy statement defines the specific method by which SESHA Fellowship Awards are selected and approved by the SESHA Board of Directors.
3 Selection and Approval Process
The following are recommended qualifications for an individual to be nominated to the SESHA Board of Directors for an SESHA “Fellowship”:
3.1 The individual must have distinguished himself/herself in an academic, technical or service capacity that has significantly contributed to the SESHA and/or the field of environmental health and safety in the semiconductor industry.
3.2 The nominee must be or have been an SESHA member in good standing.
3.3 This award is given for achievement and is to recognize a selected individual (living or dead). It is usually given to an individual with 10 years of achievement and membership in the SESHA. In the case of a deceased nominee, the minimum 10 year membership in the he SESHA may be waived and the award given to his/her heirs.
3.4This award is to be restricted to a maximum of three individuals per year to maintain its significance and uniqueness. However, the SESHA Board of Directors has the discretion to pass on granting any or all three of these awards in a given year, if qualified individuals are not available.
4 Who Can Nominate
4.1 Any current SESHA member can nominate an individual to the SESHA Board of Directors for the SESHA “Fellowship”. The nomination must be in writing and include the nominee’s qualifications and the reasons why he/she is to be considered for this prestigious award. The SESHA Board of Directors may also appoint a nominating committee to seek qualified individuals for this prestigious award.
5 Responsibilities
5.1 The SESHA Board of Directors will review the qualifications, rank, rate and vote on all nominees for the SESHA “Fellowship Award” on an annual basis. The nomination will be carried if a 2/3 majority of the Board is achieved on the vote. The SESHA Board of Directors may delegate this responsibility to a sub-committee appointed by the Board.
6 The Award
6.1 The recipient of the award will receive the following:
- A recognition plaque and presentation will be made at the annual meeting by the current SESHA President.
- An article detailing the accomplishments of the award recipient will be published in the SESHA Journal.
- The Annual conference fees are waived for all SESHA Fellows.
President’s AwardSESHAEffective Date March 1998
1 Policy Statement
To recognize an individual who has performed unique and significant service to the SESHA across several years and who has made substantial contributions to the fields of environmental health and safety in the semiconductor industry.
2 Scope
The following policy statement defines the specific method by which individuals are selected for and approved by the SESHA Board of Directors for the SESHA President’s Award.
3 Selection and Approval Process
The following are recommended qualifications for an individual to be nominated to the SESHA Board of Directors for the SESHA “President’s Award.”
3.1 The individual must have performed unique and significant service to the SESHA across several years, and have made substantial contributions to the fields of environmental health and safety in the semiconductor industry.
3.2 The nominee must be or have been an SESHA member in good standing. The nominee should have served in a significant role in the organization, e.g., as an officer, member of the Board of Directors or chairman of a committee.
3.3 This award is given for service and achievement, and is to recognize a selected individual for significant contributions over a period of years.
3.4 This award is to be restricted to a maximum of three individuals per year to maintain its significance and uniqueness. However, the SESHA Board of Directors has the discretion to pass on granting any or all three of these awards in a given year, if qualified individuals are not available.
4 Who Can Nominate
4.1 Any current SESHA officer or member of the SESHA Board of Directors can nominate an individual to the SESHA Board of Directors for the SESHA “President’s Award.” The nomination must be in writing and include the nominee’s qualifications and the reasons why he/she is to be considered for this award. The SESHA Board of Directors may also appoint a nominating committee to seek individuals for this recognition.
5 Responsibilities

Genesis AwardSESHAEffective Date March 1998
1 Policy Statement
To recognize an individual or group who has created or renovated a product or service of the SESHA, which impacts the fields of environmental health and safety in the semiconductor industry.
2 Scope
The following policy statement defines the specific method by which individuals are selected for and approved by the SESHA Board of Directors for the SESHA “Genesis” Award.
3 Selection and Approval Process
The following are recommended qualifications for an individual or group to be nominated to the SESHA Board of Directors for the SESHA “Genesis” Award.
3.1 The individual or group must have created, built or substantially renovated a product or service of the SESHA, with this process having been beneficial to practitioners in the fields of environmental health and safety in the semiconductor industry.
3.2 If an individual, the nominee must be or have been an SESHA member in good standing. If a group is nominated, at least one individual in the group must be or have been an SESHA member in good standing.
3.3 This award is to be restricted to a maximum of three individuals per year to maintain its significance and uniqueness. However, the SESHA Board of Directors has the discretion to pass on granting any or all three of these awards in a given year, if qualified individuals or groups are not available.

- The name and photo of the award recipient will be published in the SESHA Journal.
Certification or Plaque of AppreciationSESHAEffective Date – January 19971 Policy Statement
To recognize an individual, company or corporation who has contributed to the SESHA or to the semiconductor industry in the fields of environmental health and safety, or who has provided service to or otherwise supported and SESHA function.
2 Scope
The following policy statement defines the specific method by which an SESHA Certificate or Plaque of Appreciation are selected and approved by the SESHA Board of Directors.
3 Selection and Approval Process
The following are recommended qualifications for an individual, company or corporation to receive an SESHA “Certificate or Plaque of Appreciation”.
3.1 The individual must have contributed to the SESHA and/or the semiconductor industry in the fields of environmental health and safety, or have provided service to or otherwise supported an SESHA function.
3.2 This certificate is given in appreciation for service to the SESHA or the semiconductor industry to recognize the selected individual, company or corporation.
4 Who Can Nominate
4.1 Any current SESHA member can nominate an individual, company or corporation to the SESHA Board of Directors for an SESHA “Certificate or Plaque of Appreciation”. The nomination may be verbal or in writing and include the reasons why the nominee is to be considered for this “Certificate or Plaque of Appreciation”. The SESHA Board of Directors may also appoint a nominating committee to seek and recommend individuals for this recognition.
5 Responsibilities
5.1 The SESHA Board of Directors or their designee(s) will review all nominees for an SESHA “Certificate or Plaque of Appreciation” on an annual basis. In general, any individual recommended by an SESHA member or by the award nominating committee will be granted this recognition, but the SESHA Board of Directors may, at its discretion, pass on granting the recognition to a given nominee if a majority of the Board recommends against granting the recognition.
6 The Award
6.1 The recipient of the Certificate or Plaque will receive the following:
A framed Certificate or Plaque of Appreciation presented by the current SESHA President at the SESHA Annual Meeting, or by a representative of the SESHA Board of Directors at another SESHA event or function.
