Five Whys is a methodology used across the globe to investigate incidents that occur– both big and small – to identify the causes of those incidents. Its broad-based appeal is that it is free and easy to use. It is also easy to manipulate and lead a team of investigators to arrive at a pre-conceived or desired outcome. Based on this potential for misuse, it may be time to look beyond Five Whys to other established or emerging root cause analysis (RCA) methodologies which may yield more objective outcomes and better risk management results.
This session will 1) provide attendees with information on the basics of root cause analysis and current best practices that can enhance their RCA efforts and 2) help attendees understand the pros and cons of various popular RCA methodologies and tools available on the market. This information will empower attendees to assess their current RCA practices to identify and address opportunities for improvement that can strengthen their organization’s ability to learn from incidents and near-misses that occur and prevent recurrence or escalation.
